Welcome to the ACE Community Gateway.  

Welcome to the ACE Community Gateway.  The following tutorial will introduce you to the Community Gateway and take you through the essential steps required to submit a referral.

The portal is available to all referrers of ACE services.

Please ensure that all your details are entered correctly during registration as these will pre-populate some elements of the referral form. 

On registration you will receive an e-mail with a link to validate your account.  Each time you try to log on you will receive an e-mail with a verification code as a security measure.

Please do not use personal e-mail addresses; for example Gmail or Yahoo.com as these may not be accepted.

All referrers will need to complete the first part of the referral form which contains essential generic fields.

Referrers should then select the service-specific form relevant to their patient. This will be one referral form per patient per service.  Many of the fields are mandatory and these are marked by an asterisk (*). 

When you click “Send” the Community Gateway Team will receive the referral. 

Any incomplete or inaccurate referrals will be sent back to the referrer by e-mail to the address registered; these will then have to be completed again. 

Each time you log onto your account you will be able to see a full list of referrals that you have submitted through the Community Gateway web portal. If you forget your account password you can click on the link “forgotten your password” and then you can reset your password by your registered e-mail.
