Tissue Viability

The Tissue Viability Service specialist clinicians support the assessment and development of care plans for patients with complex wounds being treated in the community.  The aim of the service is ensure that all community services involved in wound care offer the same high standard of evidence based care and a holistic approach which promotes patient empowerment, self-care and well-being and appropriate pain management.   The service provides education and support for community practitioners to ensure high quality wound management and works closely with the equipment service to manage the use of tissue viability equipment in the community.

When is the service provided?
Monday – Friday 09.00am-17.00 pm. Excluding Bank Holidays

How do people access the service?
Referrals by GP, Ward Nurse, Community Nurse or allied health professional.
