Early Support Discharge Service for Stroke

The Early Supported Discharge (ESD) for Stroke service provides intensive multidisciplinary specialist rehabilitation at home for people who have had mild to moderate strokes.  The ESD team works closely with the Stroke Unit at Colchester Hospital (CHUFT) and Kate Grant Rehabilitation Unit at Clacton Hospital.  Once a person who has had a stroke is medically fit, if they would benefit from intensive rehabilitation they are discharged home with support from the ESD service.  We are able to provide 45 minutes of each required therapy daily.  This could be physiotherapy, occupational therapy and/ or speech and language therapy.  Clinical support is provided by nurses and a Stroke Consultant.  A dietician is attached to the team, to provide nutritional information.  We also have a social worker to advise on any care packages required by the individual.

Who is the service provided for?
Those who have had a mild to moderate stroke and would benefit from intensive rehabilitation.  They must be registered to a GP within North East Essex and an inpatient at Colchester Hospital or Kate Grant Rehabilitation Unit at Clacton Hospital.

When is the service provided?

Monday - Friday 0830-1630.  Weekend Nursing cover is provided by the Out of Hours District Nurses.

How do you access the service?
Referrals are taken directly from the Stroke Unit and Colchester Hospital and Kate Grant Rehabilitation Unit at Clacton Hospital.
