Community Matrons

Community Matrons look after patients with long term conditions and complex needs. If you are referred to a community matron, they will work with you to assess your needs, and you and others involved in your care will be provided with a personalised care plan based on your needs.

They will use a Single Assessment Process and work collaboratively with GPs, community services, Hospitals and the voluntary sector, and also in partnership with Social Care colleagues.

Who is the service provided to?
The service is provided to patients who have significant health needs.

Where is the service provided from?
The Community Matrons are based in both Colchester and Tendring  

The service currently operates Monday to Friday between the hours of 9am and 5pm.  

How do people access the service?

Patients are referred to Community Matrons, GP’s, Hospitals, and a range of clinical and social care workers.
